A Community Habilitation Plan

Adults and youth with intellectual and developmental disabilities, ages 12 years and older in Columbia County, NY, can choose our Community Habilitation service. You can choose this service if you are eligible for OPWDD funded services and live with a caregiver or by yourself in the community.

We provide Community Habilitation in your own home or in the Columbia County community. Together with you and your family or caregiver, we create a community habilitation plan to achieve your personal goals.

With one-on-one support from a Coarc Direct Support Professional, you can learn adult education skills such as reading and writing. You can practice daily living skills important to you for increased independence, such as budgeting, shopping, cooking, and other household skills. You can enjoy social activities and events, sports, museums, and movies.

Social work and clinical supports are available to you as needed. Community Habilitation runs seven days a week except holidays. Coarc staff provides transportation.
