From the New York State Office of People with Developmental Disabilities: What's Happening

The Office for People with Developmental Disabilities is making an important improvement to the way services are coordinated for people with intellectual and developmental disabilities in New York State, known as People First Care Coordination. Under this new model, the Medicaid Service Coordination (MSC) program which is currently used to help people plan their services has been replaced by a new and improved program called Health Home Care Management.

What you need to know

Health Home Care Management will continue to provide the service coordination that people currently receive, but will also include coordination of other services, such as health care, wellness, behavioral, and mental health services through a single individualized Life Plan, which will replace the current Individualized Service Plan (ISP).
